Student Injury and Sickness Insurance Plan

Designed especially for AOSA Members:

Dear AOSA Member:

The American Optometric Student Association is pleased to present the 2007-2008 Health Insurance Plan. The AOSA Health Insurance Plan has been designed with you, the optometry student, in mind. Unlike many others, this plan has no list of doctors or hospitals that you must use. You may select the physician and hospital of your choice. Since many students are on external rotations in their fourth year, coverage is available where you are — anywhere in the world. Coverage is also for one full year, beginning and ending August 15.

This plan is offered by United Healthcare Student Resources, a division of UICI. Student Resources specializes in student health insurance plans, and currently offers similar plans to the American Medical Student Association, the American Bar Association-Law Student Division, and the National Association of Graduate-Professional Students, to name a few. The plan is underwritten by the MEGA Life and Health Insurance Company.

All members of the American Optometric Student Association who are students are eligible to enroll in the insurance Plan. Residents who are American Optometric Student Association members and who graduated from an accredited school or college of Optometry in the previous 18 months are also eligible to enroll in the Plan. The Plan is not available to residents of New York, New Jersey, Massachusetts or Oregon. New York residents may call the company at (800) 237-0903 for information on alternative coverage.

Undergraduate students must actively attend classes for at least the first 31 days after the date for which coverage is purchased. Eligible AOSA members who do enroll may also insure their Dependents. Eligible Dependents are the spouse (residing with the Insured) and unmarried children under 19 years of age who are not self-supporting. Dependent Eligibility expires concurrently with that of the Insured student.

As a health care professional, you are well aware of the rising cost of health insurance and the increasing number of students without any health care coverage. Don't jeopardize your financial future by going without health insurance! Evaluate this plan and others, and choose the best coverage for you and your family.

For more information, please contact your AOSA Trustee or call United Healthcare Student Resources toll-free at 1-800-767-0700. You may also visit the Student Resources Website at www.uhcsr.com for brochures, enrollment cards (printable using Adobe Acrobat), coverage receipts, ID cards, claims status and other services.

Sincerely,

Christopher Wolfe
President
